Position Summary:

PLAN is seeking a Social Worker who is licensed by the Commonwealth of MA at the LICSW level. The licensed social worker will provide supervision to our six social workers on staff and provide case management services to trust participants. Social workers assist beneficiaries, with input from family and other significant individuals, in developing a spending plan, or life care plan. The Social Worker will assist participants in using their funds to obtain items and services that are important to them within the guidelines of state and federal regulations. A growing part of this position will include providing more expansive case management services for individuals with disabilities. 

Duties and Responsibilities included:

Initial meeting of new trust participants to explain the trust and use of trust funds, assess participant’s living arrangements, strengths and weaknesses, and develop a spending or life care plan.


Ongoing contact with trust participants including prompt review of participant requests, and weekly decisions on disbursements


He/She will be out in the community visiting with the assigned beneficiaries as well as communicating with families, POAs and guardians.

Timely documentation that reflects the services provided and spending plan updates required.


We expect the social worker to identify community changes and opportunities such as legislation, regulations and programs that may affect our beneficiaries.


Be the Service Coordinator and develop knowledge of resources with the goal of giving participants ideas for using their funds


Maintain current knowledge of public benefits, such as Medicaid, disability benefits and housing.


Assist with program development and innovation.


The Social Worker will be an advocate for the beneficiaries and work as a team with the trust operations staff and social service staff in administration of the trust.

Qualifications:

LICSW required, with at least 5 years of experience working with the disability community or related experience

 

Excellent skills in communication and assessment in line with social work methods and techniques as well as excellent organizational skills


Able to work effectively with a variety of disciplines in a team setting.


Be familiar with resources and supports for people with disabilities.  Strong ability to problem solve and advocate for beneficiary


Ability to work independently and interdependently


Familiarity with computers and computer programs, such as Microsoft Office and Salesforce


Integrity, positive attitude and self-direction


Fluency in Spanish or another language a plus, not required


An understanding and commitment to PLAN’s mission and program
